A power of attorney is a document in which you give another person (called your “agent” or “attorney-in-fact”), the authority to act for you (called the “principal”).  This type of document is particularly useful if you are disabled, temporarily or permanently, or suffer an unexpected, serious illness.

This document can be written as broadly or narrowly as you wish or need, and can give your agent authority to make decisions about your property and finances.

WHAT YOU WILL NEED:

Before you can complete your power of attorney document, you will need to collect the following information:

  • Your name, DOB, and full address
  • The name(s) address, and phone number for the person(s) you will designate as your primary and secondary agent for your finances (you can also provide and third choice backup agent).
  • The name(s) address, and phone number for the person(s) you will designate as your primary and secondary guardian of your person (you can also provide and third choice backup agent).
  • Your preferred effective date (when do you want the power of attorney to take effect)
